Field Survey Forms

A form builder the team owns: one question at a time on the phone, live answers and breakdowns at the desk.

A form builder and survey platform for field teams. Administrators drag fields into sections, attach searchable reference lists and add conditional logic. Field staff answer one question at a time on their phone, review and submit, then start the next visit. Answers arrive live in the admin panel with totals, breakdowns by lookup field and exports to CSV or a ZIP with uploaded files.

Client delivery. The product was designed and engineered as studio work for the team that owns it.

What the client can do now.

A field team logged visits many times a day through survey tools it didn’t own. The client wanted its own form builder: quick to fill on one phone, easy for admins to change, and with answers visible the moment they arrive.

  • Build forms without a developer

Drag fields into sections, set required answers and show fields only when they apply.

  • Search long lists on a phone

Lookups backed by reusable datasets find the right venue or doctor in a few letters.

  • Fill it in fast, many times a day

One question at a time, a review screen, then straight into the next visit.

  • Collect files and photos

Uploads go to private S3 storage and open from the response.

  • Read answers as they arrive

Live totals and a breakdown for each lookup field, without refreshing.

  • Take the data anywhere

Export to CSV, or a ZIP that includes the uploaded files.

In the field

One question at a time, with lists you can search.

Field staff open the form on their phone and answer one question at a time. Lookup questions search a reference list as they type, and fields can remember their last answer on that device, so a name typed once doesn’t have to be typed again.

  • Searchable lookups and dropdown mode
  • Answers remembered on the device where it helps
  • Progress shown on every question

Review and submit

Check the answers, submit, start the next visit.

Before submitting, staff see every answer on one review screen and can go back to fix anything. After submitting, a fresh form starts for the next visit, with uploads already safely in S3.

  • Review screen before every submission
  • File and photo uploads to S3
  • Fresh form straight after submitting

At the desk

Answers and breakdowns as they arrive.

The admin panel shows completed responses and those in progress, with a breakdown for each lookup field. Admins manage forms, datasets and their team, give colleagues access to specific forms, and export everything to CSV or a ZIP with the files.

  • Live totals with Convex subscriptions
  • CSV import for reference datasets
  • Form-scoped admin access
