Diabetes Distress Assessment

A five-question PAID-5 check in twelve languages, scored on the server, with education material matched to the result, on a phone or a booth screen.

A mobile-first PAID-5 diabetes-distress assessment for a pharma client. Patients choose one of twelve languages, answer five questions, and get a server-calculated score with matching education material. A protected admin area manages versioned questionnaire content, education PDFs, insights and anonymous reports, and a kiosk mode runs the same flow on booth touch screens.

Client delivery. The product was designed and engineered as studio work for the team that owns it.

What the client can do now.

A pharma client wanted to help doctors start a conversation about diabetes distress. That meant the validated PAID-5 scale in the languages patients actually read, a result that points to something useful, and a setup the client’s team could update without a developer.

  • Ask in twelve languages

English, Hindi, Marathi, Bengali, Gujarati, Kannada, Malayalam, Odia, Punjabi, Telugu, Tamil and Assamese.

  • Trust every score

The server scores the answers and picks the interpretation. The browser never decides the result.

  • Point to the right reading

Each result opens education material for that language and level.

  • Change content safely

Questions, answers and score bands are edited as a draft and published as a new version.

  • Run it at a booth

Kiosk mode fits each step on a touch screen and replaces the system keyboard with its own keypad.

  • Read the results without identities

Insights and CSV reports by language and interpretation, with no names or phone numbers stored.

Choose a language

Twelve languages, five short questions.

The patient picks one of twelve languages, adds a few details such as age, HbA1c and diabetes duration, and is told there are five short questions about the last month. Nothing identifying is asked, and progress lives only in that browser tab, so a refresh doesn’t lose answers and closing the tab clears them.

  • Translations taken from the supplied documents
  • No name, phone or email collected
  • In-progress answers kept only in the tab

The questions

One question at a time, on any screen.

Each PAID-5 question has five answers, from Not a problem to Serious problem. On a phone it is one question per screen. On a booth display, kiosk mode fits every step without scrolling and swaps the system keyboard for an on-screen keypad, so a touch screen at an event works as well as a phone.

  • Mobile first, with landscape layouts for wide screens
  • Kiosk mode remembered per browser
  • On-screen keypad for age, HbA1c, city and duration

The result

A score the server owns, and something to read next.

The server adds up the points from the active published version and returns the score, its band and the education material for that language and level. Admins change questions, answers and bands as a draft and publish a new version, and published versions never change, so every stored result can be traced to the exact questionnaire the patient saw.

  • Interpretation and PDF resolved on the server
  • Draft and publish for all twelve translations
  • Anonymous reports with CSV export
